A self parameter in a method.

ADR-0076 made self : T the sole spelling for receivers; the legacy inout self / borrow self keyword forms are gone. The receiver kind (by-value, Ref(Self), MutRef(Self)) is determined at parse time and recorded here directly to avoid downstream stages having to re-classify the annotation.
